GENERAL DESCRIPTION
SUMMARY OF ESSENTIAL FUNCTIONS
- Completing an initial and follow-up home assessment for each client served during the grant cycle. Documentation and Reporting: Conduct each home assessment using Home for Life Design, Home Assessment with provided access from the organization. Prepare and submit required reports to funders on schedule.
- Adhere to all applicable federal, state, and local regulations and HUD requirements related to home modifications and accessibility standards.
- Develop and implement quality control procedures to ensure high-quality home modifications and customer satisfaction standards.
- Participate in Home for Life Design, Home Assessment training, and monthly Meetings.
- Conduct outreach to eligible individuals, community organizations, and social service agencies to promote program availability and benefits.
SUMMARY OF JOB QUALIFICATIONS
- Must be able to successfully pass a drug test, background check, and a driving record check.
- Must have access to reliable transportation to perform job duties, a valid CA Driver’s License, and meet the state-required amount of personal automobile liability insurance.
- Must be a certified/licensed Occupational Therapist eligible to perform home assessments and recommend modifications.
- Background in working with aging populations, ideally in a home or community-based setting.
- Familiarity with federal, state, local, and HUD guidelines regarding accessibility and home modifications.
- Ability to complete and document home assessments using tools like Home for Life Design, Home Assessment.
- Must be able to effectively communicate with project managers, contractors, clients, and other organization staff.
- Ability to prepare and submit reports on time and maintain accurate records for grant compliance.
- Self-driven professional with attention to detail, committed to empowering older adults.
- Ability to conduct outreach and promote program services to individuals and community organizations.
COMPENSATION
Pay Range: $60.00/hr. - $65.00/hr.
