GENERAL DESCRIPTION


The Older Adults Grant Occupational Therapist will be part of the home modification team and responsible for completing an initial and follow-up home assessment, recommending home modification interventions, completing and sharing home assessment reports with the project manager, and providing education and training on installed equipment. Responsibilities also entail communicating with organization staff, contractors, and clients to meet grant requirements and ensure best practices.  The ideal candidate is a motivated, detail-oriented professional committed to empowering aging people in the community.

SUMMARY OF ESSENTIAL FUNCTIONS


  • Completing an initial and follow-up home assessment for each client served during the grant cycle. Documentation and Reporting: Conduct each home assessment using Home for Life Design, Home Assessment with provided access from the organization. Prepare and submit required reports to funders on schedule.
  • Adhere to all applicable federal, state, and local regulations and HUD requirements related to home modifications and accessibility standards.
  • Develop and implement quality control procedures to ensure high-quality home modifications and customer satisfaction standards.
  • Participate in Home for Life Design, Home Assessment training, and monthly Meetings.
  • Conduct outreach to eligible individuals, community organizations, and social service agencies to promote program availability and benefits.

SUMMARY OF JOB QUALIFICATIONS


  • Must be able to successfully pass a drug test, background check, and a driving record check.
  • Must have access to reliable transportation to perform job duties, a valid CA Driver’s License, and meet the state-required amount of personal automobile liability insurance.
  • Must be a certified/licensed Occupational Therapist eligible to perform home assessments and recommend modifications.
  • Background in working with aging populations, ideally in a home or community-based setting.
  • Familiarity with federal, state, local, and HUD guidelines regarding accessibility and home modifications.
  • Ability to complete and document home assessments using tools like Home for Life Design, Home Assessment.
  • Must be able to effectively communicate with project managers, contractors, clients, and other organization staff.
  • Ability to prepare and submit reports on time and maintain accurate records for grant compliance.
  • Self-driven professional with attention to detail, committed to empowering older adults.
  • Ability to conduct outreach and promote program services to individuals and community organizations.

COMPENSATION


Pay Range: $60.00/hr. - $65.00/hr.
